To file a claim for shipment loss or damage, please download the form and instructions here and deliver the completed form by mail, fax or e-mail to our FedEx Customer Service Team. (This form is in PDF format, requiring the use of Adobe Acrobat Reader, which can be uploaded via the link here.)
