Past Requirements
Beginning Jan. 1, 2011
FedEx Ground hazardous materials shippers must use typed or computer-generated Shipper’s Hazardous Materials certification forms (OP-950) and hazardous materials shipping papers (OP-900, OP-900LL
and OP-900LG). We encourage you to use one of our electronic shipping solutions for document completion and generation, which include FedEx Ship Manager® at fedex.com, FedEx Ship Manager® Software, FedEx Ship Manager® Server and FedEx Web Services.
Beginning Jan. 10, 2011, Shipper’s Declaration for Dangerous Goods forms for all FedEx Express® dangerous goods shipments originating in the U.S. must be prepared using software with dangerous goods compliance edit checks. These software options include:
- Certain FedEx® electronic shipping solutions.
- Preapproved shipper-proprietary software.
- FedEx approved dangerous goods vendor software.
Beginning Oct. 1, 2010
Shipping documentation for all dangerous goods and hazardous materials shipments must include an emergency response information (ERI) provider, who provides 24-hour coverage in the event of emergencies, and an emergency response phone number, including area code and country code, where applicable.
When an agency or organization is designated as the ERI provider, the registered person or entity must be identified on the shipping form by name, contract number or other unique identifier. The person or entity responsible for providing detailed ERI about the shipment’s hazardous materials content is usually the offeror or shipper.
- Order the new forms — either the OP-900LL for laser printers or the OP-900LG for dot matrix printers. To order the new forms, call 1.800.GoFedEx 1.800.463.3339 and say "rep" or press 0 to speak to the first available FedEx Customer Service representative.

Not only do our electronic shipping solutions make document creation faster, more efficient and less prone to error, but by January 2011, manual forms will no longer be accepted.
