Company Information
Enter as much information as possible about your company, including company name, type, address and Dun & Bradstreet number. Required fields are noted with an (*) asterisk.

Billing Information
Let us know specific billing details about your company, such as your billing address and a contact person in your accounting department. Required fields are noted with an (*) asterisk.

Business Information
Complete this section with specifics about your business, including the company's founding date, annual sales and monthly credit required. Required fields are noted with an (*) asterisk.

Trade References Bank
Let us know the contact information for one of your bank references. Required fields are noted with an (*) asterisk.

Transportation Provider
Let us know the contact information for one of your existing transportation providers. Required fields are noted with an (*) asterisk.

Select Submit when all information is complete. Your credit application request will be applicable for both FedEx Freight and FedEx National LTL.

You will receive a (E-mail/mail/phone) response from the Credit Department.
