Entry is easy. Simply complete the form below for each child you wish to nominate and submit your entry by 17:00 CET Monday September 20, 2021. Winners will be drawn at random for each match from all eligible entries received.

Please provide the following details for the team member nominating a child 

If you’re a winner, you’ll receive prize confirmation by email. Please ensure you accurately enter an email address you monitor regularly and your mobile number, including country code. You’ll have to confirm your acceptance by the date given in the confirmation email to secure your FedEx Ball Kids experience, so please keep a look out and respond quickly. 

You can nominate as many children that meet the family relation criteria and UEFA age criteria into the competition as you like. Please complete a separate entry form for each child you want to nominate. Please do not nominate the same child twice, as duplicate nominations will be removed.

Please ensure the each nominated child has an active interest in football and will be able to fulfil the physical requirements of the role i.e. being pitch side for up to two hours in a stadium environment and able to maintain concentration throughout the game. Please enter all the information requested below accurately as it will be used to gain accreditation to enter the stadium for the selected Ball Kids

*UEFA will aim to provide FedEx Ball Kids football kits and football boots at the sizes requested, however this may not be possible in all occasions and nearest sizing available will be provided.

To be a FedEx Ball Kid, the nominated child needs to have a good understanding of football and be able to keep the game flowing. To help us understand your nominated child’s understanding of football, please select from the following options which best describe the child’s interest and involvement in football (please select as many that are applicable).

Training materials, including the key skills required to be a FedEx Ball Kid, are available on the Ball Kids entry site and will be shared with the selected winners on the day.

You can only nominate a child to be a FedEx Ball Kid at the UEFA Nations League Finals matches taking place in the capital city of your region (Milan or Turin).  As travel could be limited or not encouraged between the two regions, please only select a match or matches taking place in your own region.

Your child will be entered into the prize draw for each match you select.

Before making your selection please be aware that FedEx Ball Kids must arrive at an agreed meeting venue in the host city 6 hours before the match kick-off for a pre-match registration and briefings, before being transported to the stadium for UEFA stadium orientation and match instructions. Please consider these arrival timings, and what time the games will finish, when selecting matches.

Please be aware that attending a UEFA Nations League Finals match, including travel time to-and-from the stadium, does not constitute working hours and should be considered as the applicant’s free time. Any resulting absence from work (parent/guardian) and school/college (child) would need to be requested and authorised through your normal work/school/college absence request process. 

Travel, tips, food, drinks and other personal expenses are not included in the prize. These items cannot be processed via FedEx or TNT expenses.
