Dangerous Goods Declaration Forms
FedEx provides interactive Shipper's Declaration for Dangerous Goods form templates to help you prepare dangerous-goods shipments. There are two versions of these forms, one with vertical columns in the Nature and Quantity of Dangerous Goods section and one without. Both forms correspond to those published in the International Air Transport Association (IATA) section 8.1.9 and are approved for shipping dangerous goods.
Specific instructions for using each template are included within the template. All form templates are in PDF format. To read PDF files, you will need the free Adobe Acrobat Reader or plug-in, version 4.0 or higher.
Note: All completed forms should have red hatchings on the sides, whether pre-printed or printed via computer.

Shipper's Declaration Form for Colour Printers
The templates below are designed for printing on colour printers using plain 8 1/2" x 11" paper. If you do not have a colour printer, you should not use these templates.
Shipper's Declaration Form for Black and White Printers
FedEx also provides templates designed for use on black-and-white printers. These forms require the use of form blanks, called LZR DG DEC. These blank forms contain pre-printed red vertical hatchings. When these templates are used, they will print the Shipper's Declaration within the red hatchings.
